Returning Player Registration
Saturday, October 10th, 2009 from 9am-2pm
Saturday, October 24th, 2009 from 9am-2pm
West Boynton Park under pavilion

Returning Player Registration fee is $95 per player - no second child discount
Pre-printed registration forms will be available.
Parent signature and payment will be all that is required.

We take cash and checks.  Checks can be made out to AYSO Region 1370.
Registration and payment is recommended to be done online for faster processing.

Please print off 2 copies and sign both at the bottom before bringing them to
the registration. You can also print blank forms and fill them out by hand.

Parents be prepared to present ID upon request

New Player Registration

To make things faster for you please log onto our new clubspaces registration website where you can now pay online.  Remember, you must still come on registration day to present your registration forms along with the other requirements listed below.

New Player Registration specifics:
- Child must be present for evaluations
- Must provide a copy of child's birth certificate
- Must meet the age requirements (child must be of age by Aug 1st, 2009!)
- Must have payment present for each child you are registering

REFUND POLICY:
Players are eligible for a refund minus a $20 administration fee if the request is made to the registrar before the first game of the season.  If a uniform is not returned, an additional $35 will be deducted from the refund.

LATE REGISTRATION:
Any registration after the scheduled registration dates will be $115 per player.  This does not apply to players chosen from the wait list.

RETURNED CHECKS:
In order to cover the fee for ISF bank charges, $15 will be added to the cost of the registration fee.  Payment must be made in cash or money order.
